Ayurveda and modern medicine are not competing answers to the same question — they are answers to different questions. Modern medicine asks what is wrong and finds the specific cause. Ayurveda asks what this person's normal is and how far they have drifted from it. Both are useful; neither covers the whole ground.

Treating this as a contest produces bad decisions in both directions — people who refuse antibiotics for pneumonia, and people who dismiss a system that manages chronic complaints their own doctor has told them to live with.

Where modern medicine is irreplaceable

This list is not a concession; it is the most important paragraph here.

  • Emergencies — heart attack, stroke, severe trauma, anaphylaxis, acute abdomen
  • Serious infection — pneumonia, sepsis, meningitis, tuberculosis
  • Cancer — diagnosis, staging and treatment
  • Surgery of any kind
  • Type 1 diabetes, hypothyroidism, epilepsy — conditions requiring replacement or suppression
  • Anything needing imaging or a lab value to diagnose
  • Vaccination
  • Obstetric complications

In none of these is Ayurveda an alternative, and no competent Ayurvedic physician will suggest otherwise. Delay in this list is where harm occurs.

Where Ayurveda genuinely earns its place

  • Functional digestive disorders — irritable bowel, functional dyspepsia, chronic constipation. Territory where conventional options are limited and Ayurvedic management is strong.
  • Chronic musculoskeletal pain — where the alternative is indefinite NSAIDs with their own costs.
  • Stress, mild anxiety and sleep — routine plus ashwagandha, which has real trial data.
  • Recovery and convalescence — a category conventional medicine barely addresses.
  • Prevention and daily routine — where Ayurveda is unusually detailed.
  • Chronic skin conditions — as an adjunct in eczema and psoriasis.
  • Menstrual and menopausal complaintsShatavari and Ashoka have long records.
  • Symptoms with normal investigations — the patient told "nothing is wrong" who still feels unwell. Ayurveda has a framework for this; conventional medicine largely does not.

The honest position is not that one is better. It is that a fractured femur and a decade of bloating are different problems, and pretending one system handles both equally well helps nobody.

The evidence gap, honestly stated

Modern medicine has vastly more trial evidence. That is not in dispute. But three things are worth understanding about why:

Personalisation resists trial design. A randomised trial gives everyone in the arm the same treatment. Ayurveda's central claim is that treatment should differ by constitution — so testing "Ayurvedic treatment" as one thing tests something Ayurveda does not propose.

Nobody can patent Triphala. Phase III trials cost tens of millions and are funded by whoever will own the result. This is an economic explanation for the gap, not a scientific one.

Some of it has been tested and holds up. Ashwagandha for stress and sleep, curcumin for osteoarthritis, Triphala for constipation, ginger for nausea, oil pulling for gingivitis — all have randomised evidence. The gap is uneven, not total.

"No trial" means untested, not disproven. Both directions of overclaim are wrong here.

Combining them safely — the part that matters

Most harm at this boundary comes not from either system but from interactions nobody was told about.

The rule that prevents most of this: tell both practitioners everything you take, including anything you consider "just a herb". Take conventional and Ayurvedic medicines two hours apart. And never stop prescribed medication to try a herb — abrupt withdrawal from antihypertensives, antiepileptics or psychiatric drugs is dangerous in itself.

The heavy metal question

This deserves stating plainly rather than being buried. Some classical Ayurvedic preparations — the Rasa Shastra category, including certain Ras and Bhasma formulations — contain processed mercury, lead and arsenic. Properly made Bhasmas undergo extensive purification, but lead and mercury poisoning traced to Ayurvedic products is documented in peer-reviewed case series.

You cannot verify correct processing by looking at a tablet. Buy this category only from licensed GMP-certified manufacturers, avoid it in pregnancy and childhood, and tell your doctor you are taking it. Purely herbal formulations do not carry this risk.

How to actually use both

  1. Get the diagnosis from conventional medicine. Imaging, bloods and a named condition. This is what it is best at.
  2. Treat acute and dangerous things conventionally. Without hesitation or delay.
  3. Use Ayurveda for the chronic, functional and preventive layer, and for recovery.
  4. Keep both practitioners informed. Every time.
  5. Keep monitoring objectively. If you take an Ayurvedic drug for blood sugar or blood pressure, keep measuring — the numbers settle the question either way.

Three reasons: personalised treatment does not fit trial designs built for standardised drugs; formulations contain dozens of ingredients, making isolation expensive; and nobody can patent Triphala, so nobody funds a phase III trial. The gap is largely economic.

Any emergency, serious infection, suspected cancer, anything needing surgery or imaging, type 1 diabetes, hypothyroidism, epilepsy, and obstetric complications. In none of these is Ayurveda an alternative, and delay is where harm occurs.

Purely herbal formulations from licensed manufacturers generally are, provided interactions are checked. Metal-containing Rasa Shastra preparations are a separate matter — heavy-metal poisoning from poorly made products is documented, so buy only from licensed sources and tell your doctor.

It manages many chronic conditions well — functional bowel disorders, chronic joint pain, stress-related insomnia, recurrent skin conditions. "Cure" is the wrong word for most of these in either system. Be sceptical of any cure claim for diabetes, cancer or autoimmune disease.

Yes, always, including things you think of as "just herbs". Several interact with common prescriptions, and some affect surgery and clotting. This single habit prevents most of the harm that occurs where the two systems meet.

Modern medicine, usually by a wide margin — hours for an antibiotic or analgesic. Ayurvedic treatment works over weeks to months, because it aims at underlying conditions rather than suppressing a symptom. That is a real trade-off, not a defect in either.
